Mission Statement

To provide services and support that allow our clients to remain as independent as possible in their homes or residences while enabling a quality of life for our clients and their families which cannot be achieved on their own.

Vision Statement

To be the leading provider of compassionate and personalized home care and hospice services, empowering individuals and families to live their lives to the fullest with dignity, comfort, and peace of mind.

In the self-advocacy community, there is a saying thaht we hold very dear to our hearts, and that is “nothing about us without us.” this means that if there is a decision of any kind about us, we except to be part of the conversation; even to lead the conversation! -Liz Weintraub, Senior Advocacy Specialtist, AUCD

  • What are the 3 types of advocacy? There are three types of advocacy – self-advocacy, individual advocacy and systems advocacy.
  • What does advocacy mean in healthcare? A patient advocate helps patients communicate with their healthcare providers so they get the information they need to make decisions about their health care. Patient advocates may also help patients set up appointments for doctor visits and medical tests and get financial, legal, and social support.
AHC does not have the typical “warm body – send anyone” caregivers. After your in-home assessment, you will be matched with a qualified caregiver that meets all your requests and needs. This is a sample caregiver profile that you would receive prior to day one for review who has the honor to assist you.
